What does it mean to be an Age-Friendly Health System?

An Age-Friendly Health System promotes dignity, respect, and person-centered care for older adults.

It means actively engaging in your healthcare journey, prioritizing holistic well-being, and collaborating with your healthcare team to achieve the best possible outcomes.

Trillium Health is proud to be an Age-Friendly Health System participant!


The Age-Friendly Health System (AFHS) program was developed by The John A. Hartford Foundation and the Institute for Healthcare Improvement. It emphasizes the "4Ms" approach: What Matters, Medication, Mentation, and Mobility.

What Matters
Understanding and respecting the values, preferences, and goals of an older adult. It's about treating each patient as an individual with unique needs and desires.

Medication
Ensuring that medications are appropriate, safe, and effective, and that the older adult understands their medications and can take them as prescribed.

Mentation
Focusing on the mental health and cognitive well-being of older adults, which includes identifying and addressing conditions like depression, anxiety, and dementia.

Mobility
Promoting mobility and independence by addressing issues that could hinder an older adult's ability to move and engage in daily activities.
