We’re excited to share that Trillium Health has expanded with a new health center at 924 Jefferson Avenue, expanding access to healthcare and supportive services for individuals and families in Southwest Rochester.
The new health center features six exam rooms and will offer:
- Family Medicine
- Behavioral Health Services
- Support Services
Patients at the Jefferson Avenue location will also have access to Trillium Health’s broader programs and services, including free home delivery of medications and assistance with insurance enrollment, food, housing, and transportation.
This new location is Trillium Health’s third medical site and reflects our continued commitment to making compassionate, judgment-free, and affordable care more accessible to the communities we serve.
