Trillium Health has been awarded a $400,000 grant from the Gilead Foundation to implement a two-year, community-based HIV prevention program in Monroe County, NY. This initiative leverages Community Health Workers (CHWs) to expand outreach, testing, and linkage to care for populations most at risk for HIV, with a focus on low-income individuals, people of Hispanic descent, and young adults.
Through community-based outreach, CHWs will provide education, rapid HIV and STI testing, and direct referrals to prevention and care services in settings such as shelters, transitional housing, and other community locations. The program also takes a holistic approach to healthcare by helping participants connect to services such as health insurance, transportation, housing, and ongoing medical care.
The initiative is designed to reach thousands of community members each year, increase access to Pre-Exposure Prophylaxis (PrEP) and other prevention services, re-engage individuals in HIV care, and strengthen connections to supportive services. Outreach efforts will include partnerships with local organizations, community events, and targeted communications to ensure the program reaches those at highest risk.
This grant also supports long-term organizational capacity by investing in CHW certification, workforce development, and infrastructure that will allow CHWs to be sustainably integrated into Trillium Health’s service model. Program evaluation will be led by Trillium Health’s Research Enterprise team, with results shared with community stakeholders to inform future HIV prevention efforts.
