Trillium Health is so much more than a health center. Compassionate, individualized care for your total well-being.

It is hard for many to see, but a big gap still separates the LGBTQ community from access to the right health and wellness programs. Just being socially accepted is not enough – personalized care is a must.

We have not conquered HIV infections among MSM. Transgender people are in need of nutrition insight, counseling and ongoing hormone therapy care. And the needs of lesbians are different than what standard practices offer.

But the Trillium Health model of care is bridging that gap. By addressing the physical, social, and emotional aspects of a person’s health, we have created a welcoming place where all lesbian, gay, bisexual, transgender, and queer people can get care that is safe, sensitive, and specialized.

Where can I get my medications?

The Pharmacy at Trillium Health is conveniently located on-site and staffed by knowledgeable, LGBTQ-affirming staff. Our full-time Clinical Pharmacist can assist with all medications.

What insurances do you accept?

We accept all major insurance plans, including state and federal programs such as ADAP, Medicaid, and Medicare. We also have a full-time health insurance assistor on staff to help guide uninsured patients in selecting a health plan.

Our Care Coordinator:

As a part of the LGBTQ Care Coordination Program, you will engage in realistic goal-setting that focuses on your personal goals and priorities. After you and the Care Coordinator work together to identify these goals, the CC will provide follow up and support to assist you in reaching each objective you’ve set for yourself. They will work with you, and in line with, your personal goals, strengths, and needs to help you achieve the best solutions to the challenges you are facing.
