Trillium Health is committed to upholding our mission by delivering extraordinary, affordable, equitable care and improving access to our care in innovative ways to draw more patients from all communities of people including Black, Indigenous, and People of Color (BIPOC) communities. There are several efforts underway:
C.A.R.E. Rochester Coalition
Trillium Health worked alongside community partners to establish the C.A.R.E. (Community, Action, Resources, and Education) Rochester Coalition with the Father Tracy Advocacy Center, IBERO American Action League, City Council, Scrantom Street Block Club, and other neighborhood groups to help revitalize the North Clinton neighborhood. We recognize that any encounter is an opportunity to provide multiple clinical and public health services for people who inject drugs, especially during COVID-19.
Empire State Supportive Housing Initiative
Trillium Health has partnered with Home Leasing to construct a new building on the corner of Union Street and East Avenue to provide high quality subsidized housing and care management services for individuals with housing insecurity as part of the Empire State Supportive Housing Initiative (ESSHI) grant we received from New York State.
Housing Opportunities for Persons With AIDS (HOPWA)
Trillium Health provides housing assistance and related supportive services to meet the housing needs of low-income people living with HIV/AIDS.
HIV Prevention and Treatment Services
Trillium Health remains designated as the western NY Ryan White provider and we continue to expand our HIV prevention and treatment services to low income individuals living with or at risk for HIV. Ryan White providers' comprehensive care approach for people living with HIV/ AIDS, results in viral load suppression, minimizing the risk of HIV transmission.
Food Security
Our food cupboard team continues to offer a stable food supply for people in need. Once a month, patients and clients can select fresh fruits and vegetables, canned goods, frozen foods, and more.
Mobile Clinic
Providing inclusive and barrier-free care for all is at the heart of Trillium Health’s mission. The Mobile Clinic allows us to bring our medical providers right into the community – helping to reach people who may have a difficult time getting to our clinic.
