the Trillium Health story

We began as a small HIV/AIDS clinic in the 1980's during difficult times.

Today, Trillium Health is a Community Health Center that provides primary care to everyone in the Greater Rochester community.

We invite you to learn more about our history of courage, hope, and innovation.


1983 | AIDS Rochester

AIDS Rochester provides advocacy, education and support services for people living with HIV/AIDS and their families.

1989 | Community Health Network

Community Health Network is established as a community-based clinic to care for people living with HIV/AIDS.

1990 | Ryan White HIV Care Act Grantee

Community Health Network becomes a Ryan White Grantee, ensuring that finances are never a barrier to receiving care.

1994 | Syringe Exchange Program

New York State's syringe exchange program is established to reduce the spread of HIV and Hepatitis C, and to connect people to healthcare and other services.

2005 | AIDS Community Health Center

Community Health Network re-brands as AIDS Community Health Center.

2005 | Pleasant Street Apothecary

Pleasant Street Apothecary joins forces with AIDS Community Health Center to ensure access to prescription drugs for HIV/AIDS - a precursor for the Pharmacy at Trillium Health.

2010 | AIDS Care

In 2010, AIDS Rochester and Community Health Network merge, becoming AIDS Care.

2013 | Trillium Health

AIDS Care re-brands as Trillium Health - broadening its services to include primary care, while retaining its legacy as an infectious disease specialty clinic and healthcare provider. The Trillium flower has three petals, symbolizing the physical, emotional and social health of its patients.

2014 | End the HIV Epidemic 2020

New York State’s Governor Andrew Cuomo announces a statewide initiative to End the HIV Epidemic by the year 2020. Trillium Health becomes an active participant in this important goal.

2015 | The MOCHA Center

The MOCHA Center, joins the Trillium Health family.

2015 | Preparation for Value-based Reimbursement

Trillium Health joins Accountable Health Partners to help reduce costs and improve patient outcomes in preparation for new health care reimbursement models.

2016 | Federally Qualified Health Center Look-Alike Designation

In 2016, Trillium Health achieves an FQHC Look-Alike designation, expanding access to primary care and supportive services.

2018 | Women's Health

Trillium Health acquires Partners in Women’s Health and Lake Affect Obstetrics & Gynecology, and establishes the Center for Gynecological Care & Wellness.

2018 | Specialty Pharmacy Designation Received

Trillium Health's Pharmacy achieves a specialty pharmacy designation, ensuring that patients continue to receive their prescriptions as part of our one-stop-shop care delivery model.

2018 | New Branding Introduced

New logos for Trillium Health and The MOCHA Center are unveiled.

2019 | Pathway Pediatrics

Trillium Health acquires Pathway Pediatrics and provides healthcare services to children and young adults.

2019 | 170 Science Parkway

A new medical facility, housing the Center for Gynecological Care & Wellness and Pathway Pediatrics, opens at 170 Science Parkway.

2019 | Pharmacy Expands

Trillium Health opens two new pharmacy locations at Monroe Avenue and Science Parkway.

2019 | Mobile Access Clinic

Trillium Health establishes a mobile access clinic to bring primary and specialty care services directly into the community.

2021 | Union Square Opens

Trillium opens 21 units at Union Square to serve individuals who are homeless and living with HIV or are age 55+ and require assistance with one or more Instrumental Activities of Daily Living.

2024 | Pediatrics and Gynecology move under the Trillium Health brand

Pathway Pediatrics and the Center for Gynecological Care and Wellness change their names and move under the Trillium Health brand.

2025 | FQHC Designation

Trillium Health becomes a Federally Qualified Health Center, increasing our access to federal resources and allowing us to serve more people
