Trillium Health Recognized as an Age-Friendly Health System Participant

Aug 7, 2023

August 7, 2023 (Rochester, NY) – Trillium Health, a community health center dedicated to delivering exceptional and inclusive care, has been recognized as an Age-Friendly Health System Participant by the Institute for Healthcare Improvement (IHI). This recognition highlights Trillium Health’s unwavering commitment to providing compassionate, patient-centered healthcare to individuals of all ages, particularly older adults.

“Trillium Health is proud to provide extraordinary care to all,” said Jason Barnecut-Kearns, President and CEO of Trillium Health. “The three petals of the Trillium Health flower represent our commitment to the social, emotional, and physical health of our patients. We believe in patient-centered care that ensures that everyone will be treated with dignity and respect.”

“We treat every person who walks through our doors as a unique person deserving of extraordinary healthcare,” said Dr. Sarah Bolduc, SVP of Clinical Services and Chief Medical Officer at Trillium Health. “We’re proud to be recognized as a place where older adults can receive the comprehensive, compassionate, and age-sensitive care they deserve."

“Trillium Health has always been driven by forward thinking, which is why we are excited to join 2,700 health systems across the globe to share data and age-friendly best practices,” said Dr. William Valenti, SVP, co-founder, and staff physician at Trillium Health. “This is a win for all of our patients, including many of our patients who are aging and living with HIV.”

The Age-Friendly Health System framework, developed by leading healthcare organizations and experts, emphasizes the "4Ms" approach: What Matters, Medication, Mentation, and Mobility.

  • What Matters: Know and align care with each older adult's specific health outcome goals and care preferences.
  • Medication: If medication is necessary, use age-friendly medications that do not interfere with What Matters to the older adult, Mobility, or Mentation across settings of care.
  • Mentation: Prevent, identify, treat, and manage dementia, depression, and delirium across settings of care.
  • Mobility: Ensure that older adults move safely every day in order to maintain function and do What Matters.

The Age-Friendly Health Systems initiative was launched in 2017 by The John A. Hartford Foundation and IHI, in partnership with the American Hospital Association and Catholic Health Association of the US. For more information, visit
