Expands access to affordable, judgment-free care in Southwest Rochester
October 2, 2026 (ROCHESTER, NY) – Trillium Health is celebrating the ribbon cutting of its newest health center at 924 Jefferson Avenue, marking an important milestone for the organization and expanding access to healthcare and supportive services in Southwest Rochester.
The new health center features six exam rooms and will offer family medicine, behavioral health, and supportive services. In addition to medical care, patients will have seamless access to Trillium Health’s comprehensive programs and services, including free home delivery of medications and assistance with insurance enrollment, food, housing, and transportation.
“The Jefferson Avenue location is Trillium Health’s third medical site and reflects the organization’s mission to provide compassionate, judgment-free, and affordable care,” said Jason Barnecut-Kearns, President and CEO of Trillium Health. “We are excited to join the Jefferson Avenue community, and we hope this location will make it easier for families to access the healthcare and supportive services they need closer to home.”
Trillium Health officially became a Federally Qualified Health Center (FQHC) on January 1, 2025, after successfully winning a federal service area competition. As part of that designation, the organization committed to expanding healthcare access and serving more patients throughout the Greater Rochester area.
The Jefferson Avenue health center is expected to serve approximately 3,500 additional patients, increasing Trillium Health’s reach beyond the nearly 16,000 patients it currently serves annually. The Health Resources and Services Administration (HRSA) has identified ZIP Code 14611 as a Medically Underserved Area (MUA).
“Community health centers provide affordable, high-quality primary care to everyone, regardless of their ability to pay,” added Jason Barnecut-Kearns. “This investment in Jefferson Avenue reflects our commitment to the health and well-being of our community. We are proud to become part of this neighborhood and offer a welcoming place where individuals and families can access the care and support they need.”
