ROCHESTER, N.Y. (May 10, 2018) — The Pathfinder Award for Team Excellence was presented to Trillium Health at the 12th Annual GRQC Showcase and Award Ceremony held last night at Locust Hill Country Club. Through the award program, the Greater Rochester Quality Council (GRQC) seeks to discover, recognize and learn from high performing organizations in the Greater Rochester area.
Focusing on expanding access, increasing census, improving patient satisfaction, improving staff engagement, and continuing to improve health outcomes, Trillium Health was recognized for the redesign and improvement of care delivery for patients.
The enhancement was intended to develop a patient care model that addressed:
- Patient access
- Patient satisfaction
- Provider and non-provider roles and staffing requirements
- Utilization of patient care teams
- Clinical management
- Quality management
- Data management
- Use of technology-optimization of Electronic Health Record (EHR)
- Clinic workflows
Finalists representing businesses, not-for-profits, and healthcare organizations were selected for their applications in three categories: Operations Excellence, Customer Excellence, and Team Excellence. Also honored were the Monroe County Medical Society, Hillside Family of Agencies, Butler/Till, Rochester Regional Health and UR Medical Center, among others.
