Trillium Health is a community-based health center offering affordable, high-quality primary and specialty care, including LGBTQ-affirming health care. Everyone who comes to us is treated with respect and dignity. Our medical team, pharmacists, health educators, and care managers are here to listen, build a relationship, and provide you with outstanding care and services. We believe in health care you can afford, and help when you can’t.

Our main location at 259 Monroe Avenue in downtown Rochester, at the corner of South Union Street, is accessible and easy to get to, with free parking and an RTS bus stop right at our front door. Recently we opened our newest location at 170 Science Parkway, home to Pathway Pediatrics, a division of Trillium Health, and our new Center for Gynecological Care & Wellness. Our Rochester and Buffalo MOCHA Centers offer programming and support for LGBTQ people of color. See the reverse side for information on all our locations.
