Trillium Health’s Day Health Program is a state-of-the-art environment that provides comfort, safety and independence through a full range of nursing, nutritional, case management, rehabilitative services and
social activities.

Our professional staff takes a comprehensive approach to developing an individualized plan of care for each participant. We assist in the management of the many challenging situations faced by persons living with HIV/AIDS. At the same time, we partner with the patients’ service providers to ensure seamless and individualized services.

Ultimately, the program provides a positive, community-based experience that is intended to improve a participant’s overall health.

What type of services can I expect at Trillium Health’s Day Health Program?

    A positive, community-based experience that is intended to improve a participant's overall health.

    We care for individuals with complex, extensive and challenging physical and social needs. Additionally, we offer participant-centered programs that are focused on helping individuals living with HIV or AIDS, who may have behavioral health problems and other chronic illnesses, lead healthier lives.

    Trillium Health’s Day Health Program serves a light breakfast daily on Monday, Tuesday, Thursday, and Friday, and a hot lunch on Tuesday and Thursday. Transportation assistance is provided.

    Our services are available to Medicaid eligible persons. Applicants for the Day Health Program must be at least 18 years old and require HIV/AIDS medical monitoring.



    Will there be someone to help with care coordination at Trillium Health’s Day Health Program?

    Individualized care and a participant-centered approach is the foundation of the program.

    Every participant is assigned a case manager who will work with clients to develop an individualized plan of services to best meet their needs.
